29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  MAIN WASH PAD:  On or about February 2, 2015 employees were exposed to the hazard of a chemical splash while utilizing corrosive liquids such as, but not limited to, Cougar Citra Brite Aluminum Cleaner (Phosphoric Acid, Sulfuric Acid) without access to suitable emergency eyewash.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  BREAK SHED and PUMP HOUSE SHED: On or about February 2, 2015, and at times prior thereto, employees were exposed to the hazard of an electrical shock of 120 volts, 16 amps from an American Power Conversion Personal Surge Arrest, Model U94042426575, outside its UL listing in that it was connected in series from the break shed with two 25 foot outdoor extension cords, across the yard, to the pump house shed.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were run through doorways, windows or floors while in use:  BREAK SHED and PUMP HOUSE SHED:  On or about February 2, 2015, and at times prior thereto, employees were exposed to the hazard of an electrical shock of 120 volts, 16 amps and fire hazard, while using an American Power Conversion Personal Surge Arrest, Model U94042426575, connected in series from the break shed with two 25 foot outdoor extension cords, across the yard, to the pump house shed which were subject to physical damage by pinch points from doorways.
