29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:   (a) On or about January 28th, 2015, employees were exposed to possible injuries in the event of an emergency in that certain areas of the plant lacked signs indicating the direction of travel to the nearest exit, and the direction of travel to the exit or exit discharge was not immediately apparent.  This condition was noted in several areas of the plant including, but not limited to, the Rough-In Department (also referred to as the Pre-Assembly Area).   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:   (a) On or about January 28th, 2015, employees operating, maintaining and/or servicing machines in the production areas of the plant were exposed to potential injuries from unexpected machine start-ups in that the employer had not developed clear and specific procedural steps for isolating each machine from its energy source(s).  These machines included, but were not necessarily limited to, the "Panel Glue Wheel" machine and the "Raimann Gang Rip" machine in the Mill Shop.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified at 29 CFR 1910.219(m) and (o):   (a) On or about January 28th, 2015, employees in the Mill Shop were exposed to possible amputation-type injuries in that the vertical belts and pulleys arrayed along the back side of the Raimann Gang Rip Saw were unguarded and easily accessible.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. 29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:   (a) On or about January 28th, 2015, employees in the Mill Shop area of the plant were exposed to potential amputation hazards due to the existence of a number of unguarded chains and sprockets on several machines used in production.  The machines included, but were not necessarily limited to, the Panel Glue Wheel machine and the Raimann Gang Rip Saw.   Abatement certification is required for this Item.
