OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards.   a) Jobsite: Mushroom Houses, 2nd level - Floor openings were not guarded by standard railings consisting of top rails, intermediate rails, and posts on all open sides, with the exception of the entrance to the stairways, exposing employees harvesting mushrooms to fall hazards.  The floor openings measured approximately 6 feet in length, 2 feet in width, with fall distances of approximately 7 feet.   Abatement Note: Among other methods, one feasible and acceptable means of abatement would be to install standard railings.   b) Jobsite: Mushroom Houses, 2nd level, right back alley to left back alley - Planks left in the rolled up position created gaps of approximately 9 - 20 inches exposing employees harvesting mushrooms to fall hazards.  An employee suffered serious injuries after falling through an opening/gap in the center alley used for ventilation purposes.  Abatement Note: Among other methods, a feasible and acceptable means of abatement would be to fully plank the walking/working surfaces with gaps measuring no more than 1 inch for ventilation purposes.   c) Jobsite: Mushroom Houses - Employees were exposed to fall hazards while using the staircases without handrails. Employees used the staircases to access the 2nd levels located approximately 7 feet above ground level.  Abatement Note: Among other methods, a feasible and acceptable means of abatement would be to install standard staircase railings.   Observed on or about 2/3/15.
