29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) Roof of the house at 5505 Landover Way, Austell, GA: Employees were exposed to fall hazards while improperly using a carabineers with positioning belt and an Ascension rope grab located at the wearer's waist.  The employees were installing shingles on a combined 2:12 to 10:12 pitched roof at a height of approximately 9 feet 8 inches from the eave to the lower level.
