29 CFR 1910.178(a)(5): The powered industrial truck was equipped with a front-end attachment other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachment and show the weight of the truck and attachment combination, and show the capacity of the truck with attachment at maximum elevation with load laterally centered:  Establishment - Toyota lift truck #9, model 7FGU25, serial number 82078, was used with the T&S Equipment Company boom attachment and was not marked to identify the attachment, show the weight of the truck and attachment combination, and show the capacity of the truck with the attachment at maximum elevation and load laterally centered.
