29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:      (a) Kaiser Construction Company, Inc.; Site at 4700 North Fifth Street, Temple, Pennsylvania - The employer did not establish and implement a written respiratory protection program that included procedures for selecting respirators, medical evaluations, fit-testing procedures for tight-fitting respirators, procedures for cleaning, disinfecting, inspecting, repairing, discarding and maintaining the respirator, training on the respiratory hazards and on the proper use of respirators, including putting them on and removing them, any limitations of their use and their maintenance for employees who were required to wear respirators when doing any cutting or demolition work, on or about February 5, 2015.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. 29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter:  (a) Kaiser Construction Company, Inc.; Site at 4700 North Fifth Street, Temple, Pennsylvania - The employer did not provide fit testing for employees who were required to wear respirators when doing any cutting or demolition work, on or about February 5, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
