29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the  safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a) 601 East Main Street - Employees were not trained on the explanation of the safety data sheet, including the order of information and how employees could obtain and use, on or about February 5th, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(g)(8): The employer did not ensure that material safety data sheets were readily accessible to the employees in their work area during each work shift:  a) 601 East Main Street - The employer did not ensure a Material Safety Data Sheet or Safety Data Sheet was available at the worksite for "M-Guard V-88 Premium Transitional Pressure Sensitive Adhesive", on or about February 5th, 2015.  No abatement certification or documentation required.
