29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a. Production Floor- On or about February 5, 2015, the employer did not establish an energy control program for employees performing cleaning operations on equipment including but not limited to the stainless steel auger machine, thereby exposing employees to the hazard of unexpected energizing of equipment.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a. Production Floor- On or about February 5, 2015, the mixing machines rotating chain and sprocket components were exposed on the left and right bottom of machine, thereby exposing employees to hazards associated with moving parts.

On or about February 5, 2015, employer did not ensure that electrical control panel door was closed and openings in the cabinet were effectively covered, including, but not limited to the following; thereby exposing employees to shock, electrocution, and arc flash. 1. Production Floor- Control panel, 240 Volt, three Phase. 2. Production Floor- Disconnect box. 3. Production Floor- Junction Boxes on yellow platform ribbon blender mchine.
