29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) 1410 Lee Street, Charleston, WV:  An employee was exposed to a fall of approximately 20 feet from the roof to the ground below while conducting roofing activities on a second story roof with a slope of 8:12, as observed on February 3, 2015.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  1410 Lee Street, Charleston, WV:  Employees used an aluminum extension ladder to access the second story roof of the residence.  The ladder was not extended at least 3 feet above the landing.  The extension ladder was extended 1 and 1/2 feet above the roof, as observed on February 3, 2015.     Abatement Certification Required for this Item.
