A) Exterior on Scaffold / Right side of house - Employees were exposed to fall hazards of approximately 17 feet while working on a fabricated framed scaffold that was not protected by a complete guardrail system (including end rails) or any other means of fall protection in place, on or about December 16th, 2014.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:   A) Exterior, Front of house: Employees were exposed to fall hazards of approximately 13 feet while using an extension ladder to access the roof of a house. The extension ladder that was used did not extend at least 3 feet above the landing, on or about December 16th, 2014.

Winters was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(1)(1), which was contained in OSHA inspection number 314678699, citation number 1, item number 2 and was affirmed as a final order on October 17th, 2011, with respect to a workplace located at 358 Beech Street in Hackensack, NJ.

B) Exterior, Rear roof of structure: An employee was exposed to fall hazards of approximately 13 feet while engaged in siding work on a residential structure without a means of fall protection in place, on or about December 16th, 2014. Winters was previously cited for repeat violations of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314678699, citation number 1, item number 3 and was affirmed as a final order on October 17th, 2011, with respect to a workplace located at 358 Beech Street in Hackensack, NJ.
