29 CFR 1917.95(b)(1)(ii): The employer did not provide and direct the wearing of personal flotation devices for those employees working in physically limited spaces that created a hazard of falling into the water: A) East Coast Terminal docking area, on or about February 5, 2015, an employee walked on a 3 feet wide catwalk, that ran over the water and parallel to the vessel and was not guarded on one side, without wearing a personal flotation device (PFD), exposing the employees to a drowning hazard.
