29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Milling & Finishing Area - On or about 2-6-15 - Employee was exposed to a amputation/severe laceration hazard from the unguarded section of the unused portion of the Wilton horizontal band saw blade.  b)  Giddings & Lewis Machine - On or about 2-6-15 - The area behind the machine where the part tables move was not guarded because the light curtain to stop access to the area was overridden by blocking the light beam on the system, creating a crushed-by hazard.
