29 CFR 1910.23(a)(3)(i): 29 CFR 1910.23(a)(3)(i):  When a floor opening was not in use, the hinged floor opening cover was not closed or the exposed side was not guarded at both top and intermediate position by removable standard railings:  a) Compressor Room Roof: On or about 2-11-2015, employees performing regular maintenance on the roof mounted  evaporative condensers, were using the roof mounted hatch access, with a fall distance of approximately 10 feet to the next lower level stairway landing without guard rails, or a procedure to require the hatch to be closed while they were working on the roof.  Note: Abatement certification is required for this item.

29 CFR 1910.119(e)(3)(iv): The process hazard analysis did not address the consequences of the failure of engineering and administrative controls:    a) Compressor Room Roof: On or about 2-11-2015, the initial PHA completed on 6-24-2009 and the Revalidation PHA completed 08-06-2014 did not consider the hazard of trapped liquid NH3 by the isolation of EC 1 & 2 that could result in high hydrostatic pressure and the rupture of  the condensers that do not have PRVs to relieve over - pressure in the case of an upset in the system.
