(b) In the Sawmill, on or before February 19, 2015 and at times prior to, the employer did not provide an annual audiogram for a lumber stacker who was exposed to an 8-hour time-weighted average sound level of 91.6 dBA during the 451 minute sampling period, which is 2.5 times the Action Level of 85 dBA, resulting in a progressive hearing loss hazard.  (c)  In the Sawmill, on or before February 19, 2015 and at times prior to, the employer did not provide an annual audiogram for a lumber stacker who was exposed to an 8-hour time-weighted average sound level of 92.3 dBA during the 457 minute sampling period, which is 2.7 times the Action Level of 85 dBA, resulting in a progressive hearing loss hazard.  (d)  In the Sawmill, on or before February 19, 2015 and at times prior to, the employer did not provide an annual audiogram for a lumber stacker who was exposed to an 8-hour time-weighted average sound level of 91.4 dBA during the 452 minute sampling period, which is 2.5 times the Action Level of 85 dBA, resulting in a progressive hearing loss hazard.
