29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  On or about February 3, 2015, at the worksite located at 9505 Chad Colley Boulevard in Fort Smith, Arkansas, The Reserve at Chaffee Crossing Apartments, on the job-made scaffold on the northwest side of Building #2200, employee accessed and worked from a single 11.5" wide board to perform siding application tasks.  This condition exposed employees to falls from elevation.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  On or about February 3, 2015, at the worksite located at 9505 Chad Colley Boulevard in Fort Smith, Arkansas, The Reserve at Chaffee Crossing Apartments, northwest side of Building #2200, a portion of the footing on the job-made scaffold rested on a damaged wooden board.  This condition exposed employees to scaffold system collapse and falls from elevations.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about February 3, 2015, at the worksite located at 9505 Chad Colley Boulevard in Fort Smith, Arkansas, The Reserve at Chaffee Crossing Apartments, northwest side of Building #2200, standard guardrailing was not provided on the open sides and end of the job-made scaffold to protect employees accessing, dismounting and working from a single board platform performing siding application tasks.  Employees are exposed to unprotected falls of greater than 10 feet.
