29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.  On 6 Feb, 2015, employees contracted by South Side Roofing were observed working on a residential roof located at 811 Russell Blvd, Saint Louis, MO. All employees on the roof were engaged in tear off of layers of roofing material while on top of the building that measured 30 feet at the eave and an additional 10 feet to the peak with a 6/12 pitched slope, as verified by Steve Holling.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 6 Feb, 2015, employees contracted by South Side Roofing were observed working on a residential roof located at 811 Russell Blvd, Saint Louis, MO. All employees working on the roof utilized the one access ladder to move material up to the roof top throughout the day. The single access ladder did not extend 3 feet above the upper landing surface.
