29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about February 9, 2015, at the above addressed worksite, employees were exposed to the hazard of falling while accessing a work platform with a ladder that did not extend at least 3 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a.  On or about February 9, 2015, at the above addressed worksite, employees were exposed to the hazard of falling a distance of 13'-4" to ground level while working near a guardrail with top rail placed at 34" from platform floor level.    Cone & Graham Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314964008, citation number 1, item number 3 and was affirmed as a final order on October 14, 2010, with respect to a workplace located at 3602 N.  US Highway 301, Tampa, FL 33609.

On or about February 9, 2015, at the above addressed worksite, employees were exposed to the hazard of falling a distance of 13'-4" to ground level while working near guardrail with top rail placed at 34" from platform floor level.    Cone & Graham Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent 29CFR1926.501(b)(1), which was contained in OSHA inspection number 315351924, citation number 1, item number 3 and was affirmed as a final order on October 14, 2010, with respect to a workplace located at 3602 N.  US Highway 301, Tampa, FL 33609.
