29 CFR 1910.134(d)(1)(i): The employer did not select and provide an appropriate respirator based on the respiratory hazard(s) to which the worker is exposed.  On or about 9 February 2015, and at times prior thereto, a mason was cutting stone with a DeWalt Masonry Saw by using a dry method without the use of a respirator while another mason was nearby, exposing the workers to silica inhalation hazards.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 9 February 2015, and at times prior thereto, two workers performing mason work on a two tier scaffold were not wearing hard hats, exposing them to the hazard of being struck by falling tools, materials or debris to the head.

On or about 9 February 2015, and at times prior thereto, a mason was using a DeWalt Masonry Saw to cut stone pieces by using a dry method without the use of eye protection, exposing the worker to the hazard of being struck to the eye(s) by flying objects.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about 9 February 2015, and at times prior thereto, two masons were accessing a tubular welded frame scaffold to perform masonry work that had two tiers without a ladder, exposing both workers to a fall hazard of approximately 15 feet to the ground below.

29 CFR 1926.702(i)(1): Masonry saws were not guarded with a semicircular enclosure over the blade.  On or about 9 February 2015, and at times prior thereto, a mason was using a DeWalt Masonry Saw to dry cut stone pieces to size in which the guard had been wedged open, exposing the mason to an amputation hazard.
