29 CFR 1910.22(b)(1): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage had to be made: (a) Doorway into Kitchen from Area of Smokehouses 8, 9, and 10 - Employees used powered pallet jacks to take stainless steel tanks of product into the Kitchen through the doorway between the Kitchen and the aisleway outside of Smokehouses 8, 9, and 10. On the Kitchen side of the doorway, the layout of equipment and the location of the doorway made it difficult for employees to maneuver through the doorway with pallet jacks loaded with tanks, exposing employees to the hazard of getting caught between the pallet jack and a stationary object, such as a wall or piece of equipment, on or about February 9, 2015. No abatement certification or documentation required.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides and railings were not provided with a toeboard where required: (a) Smokehouses 1 through 7 - Maintenance employees worked on top of smokehouses 1 through 7 to perform activities, including, but not limited to, checking and servicing valves. The tops of these smokehouses did not have a standard railing along the open side (the side above the doors into the smokehouses), exposing employees to a 10-foot fall to a lower level, on or about February 9, 2015. No abatement certification or documentation required.

29 CFR 1910.1026(d)(1): Each employer who has a workplace or work operation covered by this section shall determine the 8-hour TWA exposure for each employee exposed to chromium (VI). This determination shall be made in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section. (a) Maintenance Shop - Employees did welding on stainless steel and worked in the Maintenance Shop where a contractor did welding on stainless steel, exposing employees to chromium (VI). The employer did not determine the 8-hour time-weighted average (TWA) exposure for each employee exposed to chromium (VI), on or about January 26, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
