The employer allowed his employee to work on and from Alum-A-Pole pump jack scaffold when the scaffold was not erected on base plates and mud sills or other adequate firm foundations exposing the employee to fall hazards.       Condition occurred on or about 02/09/15.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a) 30 Washington Road, Sayreville, NJ 08872. The employer allowed his employee to work on and from Alum-A-Pole pump jack scaffold when the scaffold had not been inspected by a competent person before each shift in that the scaffold was missing guardrails and adequate firm foundations.       Condition occurred on or about 02/09/15.     PB Progress was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(f)(3), which was contained in OSHA inspection number 313981276, citation number 1, item number 1 and was affirmed as a final order on 08/31/2010, with respect to a workplace located at 76-78 Bond Avenue, Elizabeth, NJ 07201.

The employer allowed his employee to work on and from Alum-A-Pole pump jack scaffold with no fall protection system in place in that guardrails were not installed along all open sides and ends of walking/working platforms exposing the employee to fall hazards.       Condition occurred on or about 02/09/15.      PB Progress was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 313981276, citation number 1, item number 2 and was affirmed as a final order on 08/31/2010, with respect to a workplace located at 76-78 Bond Avenue, Elizabeth, NJ 07201.
