29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end. a) Fabricated Scaffold system at Hampstead High Street: On or about February 12, 2015 and times prior thereto the employer had scaffold platforms extended over it's support by more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform was able to support employee and /or materials without tipping exposing employees to fall and struck by hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Fabricated Scaffold system at Hampstead High Street: On or about February 12, 2015 and times prior thereto the employer had a scaffold platform which was more than 2 feet above a point of access exposing employees to falls hazards when accessing the platform.
