29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary: a. The employer did not ensure that each platform unit was installed so that the space between the platform and the uprights were no more than one inch wide. This violation occurred on or about February 12, 2015 when the employer allowed employees to work on a platform that had only one plank allowing a distance of more than one inch between the platform and the uprights.

The employer did not ensure that tubular welded frames and panels were braced by cross, horizontal, or diagonal braces, or a combination thereof, which would have secured vertical members together laterally. This violation occurred on or about February 12, 2015 when the employer allowed three stucco workers to work on the third level of tubular welded frame scaffolding that were not completely braced.
