29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about February 12, 2015, employees were exposed to health hazards from not being provided with adequate training to understand the hazardous properties of chemicals in the workplace such as, but not limited to, 508 Stinger Foamy Low PH Presoak/Shampoo which contained sulfuric acid and 2-butoxyethanol, while applying the pre-soak wash to vehicles.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: Carwash entrance. On or about February 12, 2015, employee(s) were exposed to corrosive chemicals such as, but not limited to, sulfuric acid and 2-butoxyethanol, while applying the pre-soak wash to vehicles without wearing chemical splash goggles. Express Wash America, dba Cascades Carwash, was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.133(a)(1), which was contained in OSHA inspection number 934427, citation number 1, item number 2, and was affirmed as a final order on May 11, 2014, with respect to a workplace located at 2809 Pat Booker Road, Universal City, Texas, 78148.
