29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a) Stan Stone LLC at 1152 S. Ogden St., Denver, CO 80209: On and before January 28, 2015 the exposing employer did not ensure the working platforms on the fabricated frame scaffold were fully planked in that two employees were applying stucco to the exterior of a residential home while standing on only one or two planks. This condition exposed the employees to a fall of approximately 12 feet.

29 CFR 1926.451(f)(7): The scaffold was erected, moved, dismantled, or altered without the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration: (a) Stan Stone LLC at 1152 S. Ogden St., Denver, CO 80209: On and before January 28, 2015, the exposing employer did not ensure a fabricated frame scaffold was erected with the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration. A scaffold system was erected on the north and south sides of a new home under construction by non-qualified personnel. This resulted in a scaffold system that was not fully planked and utilized while in that condition by the employees to apply stucco to the exterior walls of the house. This condition exposed employees to falls of approximately 12 feet.
