Ogden St., Denver, CO 80209:  On and before January 28, 2015, the exposing employer did not ensure a fabricated frame scaffold was erected with the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration. A scaffold system was erected on the north and south sides of a new home under construction by non-qualified personnel. This resulted in a scaffold system that was not fully planked and utilized while in that condition by the employees to apply stucco to the exterior walls of the house. This condition exposed employees to falls of approximately 12 feet.
