29 CFR 1926.652(a)(2): Protective systems did not have the capacity to resist without failure all loads that were intended or could reasonably be expected to be applied or transmitted to the system:  Inside excavation: On or about February 16, 2015 employees were exposed to a struck-by and engulfment hazard from lack of adequate protection while sealing one end of the protective form with sandbags to stop entrainment of flowable fill into the work area.
