On or about 17 February 2015, and at times prior thereto, employees were allowed to bypass a system interlock in order to perform normal operations by placing cups of humus in slots of a rotating tray table, or to add oils, or to remove excess laminate from a cutting/heater element, exposing the employees to the hazard of being caught in the sealing mechanism. The interlock was on a Plexiglas door and had been unscrewed from the door, and taped to the corresponding magnetic interlock on the frame, in order to be able to open the door without the interlock stopping the packaging system.
