29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. On or about 17 February 2015, and at times prior thereto, two fire extinguishers located within the restaurant and kitchen area did not have signage to readily identify and locate the units, exposing the employees working in the area to a fire hazard.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting. On or about 17 February 2015, and at times prior thereto, fire extinguishers were provided in the kitchen and restaurant areas of the establishment, with management's expectation that employees utilize them for small incipient fires, but without the employees being trained on how to use them or under what conditions, exposing those employees to a fire hazard.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible. On or about 17 February 2015, and at times prior thereto, employees were allowed to bypass a system interlock in order to perform normal operations by placing cups of humus in slots of a rotating tray table, or to add oils, or to remove excess laminate from a cutting/heater element, exposing the employees to the hazard of being caught in the sealing mechanism. The interlock was on a Plexiglas door and had been unscrewed from the door, and taped to the corresponding magnetic interlock on the frame, in order to be able to open the door without the interlock stopping the packaging system.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening whwere they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910. On or about 17 February 2015, and at times prior thereto, an electrical breaker panel and disconnect switch were blocked by a plastic crate stored in front, exposing employees to a tripping hazard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure. On or about 17 February 2015, and at times prior thereto, a relocatable power tap was zip tied to a metal rack in a manner making the wiring permanent in nature, exposing the employees using the tap to an electrical hazard of approximately 120 volts.
