On or about February 18, 2015 and at times prior thereto, work rest(s) on the bench grinder where not adjusted to within one eighth inch on the right abrasive wheel and was completely missing from the left abrasive wheel. The employees were exposed to struck-by and caught-in hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Worksite located at 7418 East 46th Place. Tulsa, OK 74145. On or about February 18, 2015 and at times prior thereto, tongue guards on the bench grinder where missing from the grinding wheel. The employees were exposed to struck-by and caught-in hazards.

29 CFR 1910.215(d)(1): Wheel(s) on grinding machine(s) were not sounded (ring test) and inspected for damage before mounting:  Worksite located at 7418 East 46th Place. Tulsa, OK 74145. On or about February 18, 2015 and at times prior thereto, abrasive wheels were not ring tested prior to mounting on the bench grinder.  The employees were exposed to struck-by hazards.
