29 CFR 1910.269(a)(2)(i)(A): Each employee was not trained in, and familiar with, the safety-related work practices, safety procedures, and other safety requirements in this section that pertain to his or her job assignments.    On or about February 13, 2015, at an overhead powerline replacement site near Muldrow, OK, the employer did not train an employee to ensure he was familiar with safety-related work practices, safety procedures and other safety requirements pertaining to his job assignments.  The employee was exposed to the hazard of electrical shock.

29 CFR 1910.269(l)(1)(ii): Unqualified employees were allowed to work in areas containing unguarded, uninsulated energized lines or parts of equipment operating at 50 volts or more.                 On or about February 13, 2015, at an overhead powerline replacement site near Muldrow, OK, the employer did not ensure an employee was qualified to work on unguarded, uninsulated energized lines and parts of equipment.  The employee was exposed to the hazard of electrical shock.

29 CFR 1910.269(l)(3)(i): The employer did not establish minimum approach distances no less than the distances computed by Table R-3 for ac systems or Table R-8 for dc systems.  On or about February 13, 2015 and at times prior to, at an overhead powerline replacement site near Muldrow, OK,  the employer did not establish a minimum approach distance as an employee worked on energized overhead power-lines.  The employee was exposed to the hazard of electrical shock when contact was made with the energized line.
