OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards created by the use of industrial storage racks which were not anchored to the floor: a) Shop area, 6863 Tyrone Pike, Tyrone, PA 16686: Observed on or about February 18, 2015, employees were using industrial storage racks that were not anchored to the floor. The industrial storage racks were used to store steel plates. The industrial storage racks were at risk of tipping over while being loaded or unloaded with powered industrial trucks and/or an overhead crane. Employees loading the industrial storage racks and employees working in the area of the industrial storage racks were exposed to crushed-by and struck-by hazards.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Shop Area, 6863 Tyrone Pike, Tyrone, PA 16686: Observed on or about February 18, 2015, employees working on the upper levels of industrial storage racks located in the front of the shop were not provided with protective equipment to prevent them from falling off of the racks. The employees were exposed to a fall hazard of approximately 9.5 feet to the concrete floor below while inserting wedges between steel plates stored on the upper most levels of the racks.

Employees working in the shop area were exposed to struck-by hazards created by the unsecured cylinder. b) Cylinder storage area, back of main shop, 6863 Tyrone Pike, Tyrone, Pennsylvania 16686: Observed on or about February 18, 2015, compressed gas cylinders, including a cylinder of oxygen and a cylinder of acetylene, were not secured to prevent them falling or being knocked over. A flexible cable was used to carry power (208 Volt, 3 phase, 30 amp) between an electrical panel marked 'S-2' and a switch box labeled 'Temp Saw Connection.' The cable was attached to the building and run through a hole in a beam. Employees working in the area of the facility where this cable was located were exposed to fire and electrical shock hazards.
