29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.  a.  At the work site located at 765 Pierce Drive, Columbus, Ohio on the day of inspection, the employer had not recorded employee injuries and illnesses for 2015 on an OSHA 300, 300A Summary or equivalent.  b.  At the work site located at 765 Pierce Drive, Columbus, Ohio on the day of inspection, the employer had not recorded employee injuries and illnesses for 2014 on an OSHA 300, 300A or equivalent.  c.
