29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employees:    a) At 6850 US 90 14A Daphne, Alabama:  On or about February 17, 2015 and at times prior; the employer failed to ensure employees working from an elevated aerial lift basket were wearing a harness and tied off exposing them to a fall hazard of 22 feet to the concrete ground below.
