29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: a) 779 Washington St., Auburn, MA: On or about February 18, 2015, employees were riding in aerial lifts without wearing a body belt or being attached to the boom or basket in order to travel up to/down from their roof top work location which was more than 17 feet above the ground below. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a) 779 Washington St., Auburn, MA: On or about February 18, 2015, three employees were removing snow from the roof of the Home Depot at this address, more than 17 feet above the ground below, without any equipment to keep them from falling to the ground below and suffering serious injury. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
