OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  On or about 2/17/2015, at the above addressed jobsite, employees were exposed to a struck-by hazard from a truck backing up in an area with workers picking produce without the driver being able to see in the path of travel behind the vehicle.    NOTE: CERTIFICATION OF ABATEMENT MUST BE PROVIDED FOR THIS ITEM.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident.   On or about 2/17/2015, at the above addressed jobsite, The employer failed to report a work-related death in a timely manner in that, an employee died on 2/17/2015 as a result of a work related injury, and the employer reported the incident on 2/19/2015.  NOTE: CERTIFICATION OF ABATEMENT MUST BE PROVIDED FOR THIS ITEM.
