29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. (a) Kitchen - The employer did not ensure that the work areas were kept in an orderly state and free of tripping hazards. The refrigeration unit was plugged into an electrical outlet box located on the floor in the middle of the work area exposing employees to tripping hazards.

29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: (a) Kitchen - The employer did not ensure that workroom floors were maintained in a dry condition as far as possible. Water was leaking from the damaged ceiling in the kitchen at numerous locations resulting in water accumulation on the floor and exposing employees to slipping hazards.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. (a) Kitchen - The employer did not ensure that electrical equipment was free from damage that may adversely affect the safe operation of the equipment. An electrical outlet box located on the floor powering the refrigeration equipment was damaged and had burn marks on it.
