29 CFR 1915.89(c)(7)(iv):   The lockout and tags-plus systems log did not contain the location of machinery, equipment, or system being serviced.  F/V Bering Leader, Coastal Dock, Factory area:  On or about 9 February 2015 and times prior thereto, fish grinder #1 and #2 were locked out and not annotated in the lock out log.  This condition exposed employees to a caught in hazard.

29 CFR 1918.64(a): Readily accessible stop controls were not provided on powered conveyors for use in an emergency.  F/v Bering Leader, Coastal Dock, Freezer area: on or about 9 February 2015 and times prior thereto, the 20 conveyor did not have an emergency stop in close proximity of the conveyor.  Conveyor was routed through a bulk head.  this condition exposed employees to a caught in hazard.
