29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  Worksite located at Hwy 44 & Interstate 40, Exit 53. Foss, OK 73647. On or about February 19, 2015 an employee was observed working from an aerial lift without the use of a safety harness and lanyard. The employee was exposed to a fall hazard of approximately 12 feet.
