29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) On or about February 18, 2015 and times prior; battery refill area: The employer failed to ensure proper eye protection was worn when forklift batteries are filled using a water hose exposing employees to the hazards associated with acid splash back.  b) On or about February 18, 2015 and times prior; wonder gel: The employer failed to ensure proper eye protection was worn when wonder gel is applied by hand exposing employees to the hazards associated with hydrofluoric and nitric acid splash back.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about February 18, 2015 and times prior; the Tin knocker machine: The employer failed to insure proper guarding was installed to prevent the operator and at time the assistant operator from getting caught-in the in-going rollers due to excessive opening space of 1 1/8 inch which would allow fingers to make contact with the rollers set 4 inches back.
