29 CFR 1910.179(m)(1): A thorough inspection of all ropes was not made at least once a month and a certification record which included the date of the inspection, signature of the person who conducted the inspection and an identifier for the ropes which were inspected was not kept on file where readily available to appointed personnel:  a)    Tidewater Yacht Marina: On or about 01/15/2015, a thorough wire rope inspection for the Marine Travelift MDL#60-BFM, Serial #1877-184 was not performed at least monthly and certification records were not prepared.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.184(c)(1): Sling(s) that were damaged or defective were used:  a)    Tidewater Yacht Marina: On or about 01/15/2015, the nylon web slings with broken stitches and used with the Marine Travelift Model #60-BRM Serial #1877-184 were not removed from service, thus exposing employees to a potential struck-by hazard.   No abatement certification or documentation required.

29 CFR 1910.179(l)(1): A preventive maintenance program based on the crane manufacturer's recommendations was not established:  a)    Tidewater Yacht Marina: On or about 01/15/2015, a preventive maintenance program was not established for the Marine Travelift Model #60-BRM Serial #1877-184, as a result required maintenance was not conducted in accordance with the manufacturer's recommendation.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
