29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: a) F/V Alaska Mist, Factory, Deck, and Engine Spaces: On or about 2/4/2015, and times prior thereto, hearing conservation monitoring had not been completed for the vessel. This condition allowed for employees to not have the knowledge of noise hazardous conditions and where to wear hearing protection on the vessel.

This exposed employees to eye injuries from using a corrosive cleaner without any protection.
