29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  a)  F/V Alaska Mist, Factory, Deck, and Engine Spaces:  On or about 2/4/2015, and times prior thereto, hearing conservation monitoring had not been completed for the vessel.  This condition allowed for employees to not have the knowledge of noise hazardous conditions and where to wear hearing protection on the vessel.

29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  (a)  Factory:  On or about February 4, 2015 and times prior thereto, employees were cleaning with Santi 75 and Super Red Plus without wearing eye protection.  This exposed employees to eye injuries from using a corrosive cleaner without any protection.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  (a)  Factory:  On or about February 4, 2015 and times prior thereto, employees were not using gloves while using a corrosive chemical.  This exposed employees to skin absorption of chemicals and chemical burn hazard from not protecting their skin.

(a)  Reefer Shop:  On or about February 4, 2015 and times prior thereto, an air nozzle used for cleaning was operating at 90 p.s.i.  This exposed employees to laceration/air embolism hazard from using an air nozzle that would not reduce if dead-ended with skin.

This exposed employees to delays in getting proper first aid and knowing the correct PPE for the type of chemical.

The lack of training would limit their ability to understand the new requirements of the program.
