29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  Roof, 475 Washington Street, Auburn, MA. On or about 23 February: employees were observed removing snow from a roof over 25 feet above then next lower level without the required PPE to prevent employees from falling from the roof to the ground below.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
