29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: (a) On or about February 11, 2015 for the employees who were exposed to crushing hazards while working on a pipe connection next to a vertical wall measuring approximately seven feet six inches in height and did not have an adequate means of protection on the West side of Highway 23 in McKenzie County, ND.
