29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      a) Zinc Plating Department- The employer did not ensure that employees adding chemicals to the Zinc Plating Line were adequately protected from hazards created by rotating parts and ingoing nip points, such as those created by the gears of the rotating barrels.    On March 29, 2015, an employee sustained an amputation injury when his hand was caught in the ingoing nip point created by the gear of the rotating barrel.

29 CFR 1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical(s) contained therein:      a) Post Finish Department- The employer did not ensure that each container of hazardous chemicals in the workplace, including containers of Universal Sealer and Rite Guard Part A, was labeled with the identity of the hazardous chemical(s) contained therein. Hazardous chemicals in these containers include, but are not limited to:  potassium silicate, and silicic acid, lithium salt.      b) Post Finish Department- The employer did not ensure that each container of hazardous chemicals in the workplace,  including containers of Y2K Yellow (Yellow Chromate), Green Dye #5 (Green Olive), Sealer CD 7.5 (US Sealer 7.5%), Sealer CD 50 Clear (Sealer CO 5%), Olive Drab Chromate (Olive Drab), Torque'N Tension UV Fluid (Color Wash 5%), Straus Chemical Tritop Concentrate (Tritop 40), Unicote Sealer 30%, and Universal Sealer (ATF 24 Sealer), was labeled with the information regarding the hazards of the chemicals contained therein.  Hazardous chemicals in these containers include, but are not limited to:  chromic acid, sulfuric acid, acetic acid, nitric acid, polyethylene wax emulsion in water, sodium bichromate, zinc nitrate, amorphous silica, potassium silicate, and silicic acid, lithium salt.
