On March 29, 2015, an employee sustained an amputation injury when his hand was caught in the ingoing nip point created by the gear of the rotating barrel.

Hazardous chemicals in these containers include, but are not limited to:  potassium silicate, and silicic acid, lithium salt.      b) Post Finish Department- The employer did not ensure that each container of hazardous chemicals in the workplace,  including containers of Y2K Yellow (Yellow Chromate), Green Dye #5 (Green Olive), Sealer CD 7.5 (US Sealer 7.5%), Sealer CD 50 Clear (Sealer CO 5%), Olive Drab Chromate (Olive Drab), Torque'N Tension UV Fluid (Color Wash 5%), Straus Chemical Tritop Concentrate (Tritop 40), Unicote Sealer 30%, and Universal Sealer (ATF 24 Sealer), was labeled with the information regarding the hazards of the chemicals contained therein.  Hazardous chemicals in these containers include, but are not limited to:  chromic acid, sulfuric acid, acetic acid, nitric acid, polyethylene wax emulsion in water, sodium bichromate, zinc nitrate, amorphous silica, potassium silicate, and silicic acid, lithium salt.
