OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a. On or about February 24, 2015, an employee using a SJIII SkyJack scissors lift (#3219), was not protected from a tip-over hazard, in that the employee exerted side forces on the aerial platform while elevated and deploying the CAFCO 300 fire retardant spray. During the spray operations, the hose became taut, causing the employee to tip-over, resulting in an injury.      Among other methods, one feasible and acceptable method to correct this hazard is to, but not limited to the following: (1) conform to the SkyJack manufacturers guide Section 1.14 safety precautions avoid putting side force on the lift to avoid tipping the lift.
