29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Rear Right, Outside Ground Level of Structure - Two workers were working on the outside window door trim from the basket of a 40' hydraulic extended boom man lift, one worker was wearing the required personal fall restraint/arrest system while the other worker was not, on or about 2/25/2015. For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHAs Multi-Employer Workplace policy.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Top Level of Structure, Front and Rear Right Side - Workers were laying 4x8 sheets of plywood to the floor/ceiling joist not protected from falling approximately 30' to the ground below, on or about 02/25/2015. For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHAs Multi-Employer Workplace policy.
