29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. Such plates shall not be removed or defaced but, if lost, removed, or defaced, shall be replaced by the owner or his agent.     a) Potts Welding and Boiler Repair Company Inc. - The employer stored heavy equipment and supplies on the shelving throughout the production facility, the shelving was identified without an approved load rating attached to any of its structures. This was observed on February 26, 2015.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service:    a) Potts Welding and Boiler Repair Co, Inc. - The employer did not ensure that an inspection of the propane powered industrial truck [Hyster # 4] was conducted prior to being operated  by management of the facility who was later determined to have been provided formal and On The Job (OTJ) training. This was observed on February 26, 2015.

29 CFR 1910.134(c)(1): The employer did not establish and implement a written respiratory protection program with worksite-specific procedures for voluntary respirator users:   a) Potts Welding Boiler Repair Co, Inc. : The employer did not develop a written respiratory protection program for those employees who voluntarily donned,N-95 half-face, negative pressure respirators  Model #' 8210. These employees were observed wearing filter face pieces while operating the super heater to bend piping in bay # 1 of the production facility. This was observed on February 26, 2015.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:      a) Potts Welding and Boiler Repair Co Inc.: The employer did not provide employees who voluntarily donned N95, negative pressure, particulate respirators (Model #8020) with Appendix D of the respiratory protection standard. The employees voluntarily donned the respirators without being provided information regarding how it's to be use and worn and it's possible effects on the employees body. This was observed on February 26, 2015.
