(a) The employer did not ensure that a complete guardrail system surrounded a platform above the # 28 plastic injection molding machine, exposing employees to fall hazards.    (b) The employer did not ensure that a complete guardrail system surrounded a platform above the # 31 plastic injection molding machine, exposing employees to fall hazards.
