29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  (a) Warehouse, back wall - On or about 02/26/15 and at times prior thereto the on-off switch box (Furnas 3-phase Catalog No 14BF32BC) for the air fan/circulator mounted on the wall did not have a cover, exposing workers to accidental contact with live conductors and terminals.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
