29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At the jobsite, employees were exposed to a 10 foot fall hazard, in that, fall protection was not used while installing plywood on a 6:12 pitch roof. Violation observed on or about February 26, 2015.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) At the jobsite, employees were exposed to a 10 foot fall hazard, in that, an extension ladder was used by the employees to access and descend from the roof, extended approximately 6 inches above the landing surface. The ladder was also used and extended approximately 18 inches above the landing surface on another section of the property. This violation was observed on or about February 26, 2015.
