Worksite located at 8910 E. 350 Highway, Raytown, Missouri- Employees were working directly beneath elevated employee and equipment and was not protected from impact by the use of hard hats.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a). Worksite located at 8910 E. 350 Highway, Raytown, Missouri- Employees were exposed to a shock or electrocution hazard in that the employer failed to inspect equipment that had a breach in its' outer insulation and was missing a grounding pin.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  (a). Worksite located at 8910 E. 350 Highway, Raytown, Missouri- Employee was subjected to a fall hazard in that the platform being used to elevated the employee was not secured to the forks of the lifting carriage.
