29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. Grand Ronde Construction Project- The unguarded stairway landing had wall openings of 41 1/2 X 22 1/2 inches, potentially exposing workers to falls of 7'- 4 1/2" to the level below.

29 CFR 1926.501(c)(1): When employees were exposed to falling objects, the employer did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels: Grand Ronde Construction Project- (a) The open mechanical chase was not equipped with a toe board to prevent objects, such as tools and equipment from falling onto the walkway below.

29 CFR 1926.502(b)(6): Guardrail systems were not surfaced as to prevent injury to an employee from punctures or lacerations, and to prevent snagging of the clothing: Grand Ronde Construction Project- (a) The top handrail located on the 2nd level, had a nail protruding exposing workers to injuries. (b) The mid handrail located on the 2nd level had a nail protruding exposing workers to injuries.
